PagerDuty is priced per user per month, and published tiers run from roughly $21 to $39 per user on the standard plans, with higher enterprise tiers above that. Check PagerDuty's pricing page for current numbers. For a three-person team that is a meaningful annual line item, and it buys a great deal of capability most small teams never configure.
The question worth asking is which part you use. If you run rotations, track MTTA and MTTR, hold incident reviews and route by service ownership, PagerDuty is the right tool and this page is not for you.
If, on the other hand, your on-call rota is "whoever is not on holiday" and the value you get from PagerDuty is that it phones that person, then you are buying a platform to use one channel. Email2Phone is that channel on its own.

Can I keep PagerDuty and add Email2Phone?

What about acknowledging an alert?
Email2Phone does require acknowledgement: a new message is only accepted when the recipient presses Save, Delete or Reply on the keypad, and it keeps calling back until someone does. You can also set the escalation to stop at the first acknowledgement. What it does not do is record that acknowledgement in an incident timeline or report on it, which is what PagerDuty adds.
